Issues with sounds on IOS 9.2

Hi all,
I need some help here. I'm having 3 issues with sounds since I updated my iPhone 5s to IOS 9.2. First, and probably most annoying to me is that I no longer have the lock screen and keyboard click sounds. When my iPhone powers on, VO says "muted" and I'm pretty sure those sounds have been muted. Here are the things I have tried so far:
1. Reboot phone.
2. Reboot by pressing the home and power buttons.
3. Gone to settings>sounds and made sure both check boxes were checked.
4. Unchecked both boxes in settings>sounds, rebooted phone, then rechecked the boxes
5. Made sure volume was turned up.
I'm out of ideas on this one. I've looked all over settings and can't find any more options that might help.

The second issue I'm having is that no sounds are playing in games, like Dice world or Trivia Crack, when I don't have my earbuds plugged in. I can hear VO just fine on the built in speakers, but no game sounds. Music also plays over the built in speaker just fine. I've rebooted both ways mentioned before and made sure the volume was at an acceptable level, but nothing helped. Any ideas?

The third issue I'm having is with audioducking. If I turn audioducking on, all sounds dim when VO is speaking, but they never come back up when VO stops speaking. I'm fresh out of thoughts on this one. Any help would be very greatly appreciated.
